Case Management
Case Management provides services through its Traumatic Brain Injury (TBI) and Disabled and Elderly (D&E) programs, both under the auspices of the NM Aging and Long Term Services Division. Case management serves to link clients to crisis and long-term services based upon the client’s individual needs.
The Traumatic Brain Injury (TBI) Program is a crisis interim service. Its primary purpose is to protect individuals with TBI from imminent risk to their safety and health. The program assists clients with identifying, requesting and obtaining long-term services, and to provide the support and stability necessary to keep a client stable in the community. Crisis interim services are intended to assist clients in crisis with services that are directly related to the client’s TBI, such as assistive equipment, initial housing costs, environmental modifications, retrofit of an automobile, homemaker services, respite care, transportation to receive TBI medical care, therapy services, prescription medications and life skills coaching.
The Disabled & Elderly Waiver (D&E) Program is a Medicaid-funded home and community-based waiver service program for disabled and elderly persons. Services are intended to assist clients in remaining in the community. Services available to D&E clients are adult day health, assisted living, emergency response, homemaker services, occupational, speech and physical therapy, case management, environmental modifications, respite and private duty nursing.
