    ​The next step of the selection process is to complete this follow-up questionnaire.

    Questions 1-6 are required.  Questions 7-11 are voluntary and will be used to help evaluate the inclusiveness of our training. 
    ​If you would prefer not to answer questions 7-11, please answer with "Prefer not to answer" or N/A.  All questions must be completed.  
    Terms of Program Participation
    Goodwill Industries of New Mexico (GINM) has the right to refuse service to program participants.     • Participants will only be considered after the successful completion of a phone interview.
    • Due to limited capacity, the successful completion of the interview does not guarantee you will be selected for the training,
    • Successful completion of the training does not guarantee employment with a production.

    I give GINM permission to share my PA resume with productions, 1st Assistant Directors (AD), and (or) 2nd Assistant Directors.
